2011 Summer Champs Meet Entry Sign Up Form

Form is used by Pocono Piranhas Team Members for Champs only

Please follow the steps below to sign up: Fill Out the below form using the PDF event info (Champs) Press Enter Your Events Make a check out to (PFY Swim Team) for the total amount due for your entries. Write in the Memo part of check (Summer 2011 Champs) Mail Check to: Sonia Wolbert 277 Prospect Street East Stroudsburg, PA. 18301 Make sure you make out separate checks for each meet you sign up for. Entries are due 7/29/11 Meet Information in PDF

All Swimmers are required to pay for 5 events ($18.00) If a relay can not be created due to lack of swimmers in that age group no refund will be given KEEP IN MIND ALL TOP SEEDED RELAYS WILL BE MADE UP OF SWIMMER WHO HAVE THE 4 FASTEST TIMES (THIS CAN BE CHANGED AT ANY TIME BY THE HEAD COACH)
WORKER POSITIONS WILL BE ASSIGNED BY MEET DIRECTOR AND POSTED THE WEEK OF CHAMPS. IF YOU HAVE ANY PREFERENCE OR LIMITATIONS PLEASE INDICATE AND WE WILL TRY TO ACCOMMODATE REQUESTS BUT CANNOT GUARANTEE THEM.
