PFY 2012 Penn Del Champs Meet Entry Sign Up Form

Form is used by Pocono Piranhas Team Members for Champs only

Please follow the steps below to sign up: Fill Out the below form using the PDF event info (Girls Champs or Boys Champs) Press Enter Your Events Make a check out to (PFY Swim Team) for the total amount due for your entries. Write in the Memo part of check (Girls Champs or Boys Champs) Mail Check to: Sonia Wolbert 277 Prospect Street East Stroudsburg, PA. 18301 Make sure you make out separate checks for each meet you sign up for. Girls Entries are due 2/6/12 - $3.00 per event Boys Entries are due 2/6/12 - $3.00 per event

All Swimmers are required to pay for both relays ($6.00) If a relay can not be created due to lack of swimmers in that age group a refund will be given KEEP IN MIND ALL TOP SEEDED RELAYS MAY BE MADE UP OF THE 4 FASTEST SWIMMERS PER RELAY

Any Questions or Comments

If a family has a past due for meet entry fees or any other
payment due, their child will no be able to swim in meets or in
practices until the balance is paid in full.
